Q1. The example mentioned by Paul during the CLMA presentation where they create an order code in Sunquest which prompts an exception in the billing system and the claim will be cleared.  Are they performing the test when this code is used? If so, are they giving the results to the physicians?

Q2. What is the impact on states that allow patients to order their own lab tests?

Q3. It was mentioned that a verbal order can simply be documented in the chart. We had a situation where the verbal order was documented and sent to the physician for signature. Because the physician signed it beyond 48-hours after the original order date, the order was deemed to be invalid. Would we have fared better not to have sought the physician signature and merely documented the verbal order in the chart as having been confirmed with the physician?

Q2. What is the impact on states that allow patients to order their own lab tests?

Since this affect only Medicare and Medicaid patients and neither of these programs reimburses for test not ordered by a physician or on physician practitioner (PA, NPP etc.), there is no impact f self ordered tests.

Yes, for the purposes of this rule, you were only required to document the verbal order in your records.  CLIA or state requirements may involves seeking a written order but that would not need to be signed and does not realte to this rule's requirements.

Yes to both questions. We are running the test and reporting. The error notifies our billing personnel that a signature is missing and they then do the follow-up work to obtain a signature. If this become a hugh financial loss to us because of not billing for unsigned requisitions, we may have to revisit our decission to run and report in the future.
