Module 1: Ripped From the Headlines!

Session Summary:

Some laboratory stories are truly ripped from the headlines—certainly not the kind of story that you would want to be written about your laboratory! Nonconforming events abound in our laboratories. We owe it to our patients to do something about them. This web conference provides a brief review of the important elements of a laboratory nonconforming event management program and how information gleaned from it will improve your laboratory’s contribution to patient safety.

 

Learning Objectives

At the conclusion of this series, participants should be able to:

  • List the 7 major elements of a laboratory nonconforming event management program
  • Define the concept of “drift” into an error-prone state
  • Describe why non-automated laboratory processes have difficulty achieving error rates of less then 1 error in 1000 opportunities
  • Describe one strong and one weak intervention to reduce or eliminate an error

 

Presenter:  Dr. Michael Astion

Dr. Astion is currently a clinical pathologist who is a Professor and Director of Reference Laboratory Services in the University of Washington Department of Laboratory Medicine.  He was an undergraduate at Cornell University and received his M.D. and Ph.D. degrees from the University of Pennsylvania under the auspices of the NIH Medical Scientist Training Program.   He has been on the faculty at the University of Washington since 1993. He is a senior member of a physician management team that oversees a laboratory with more than 1000 employees. He has authored 20 educational software titles and 40 peer-reviewed papers covering a number of topics in laboratory medicine. The software titles serve as the basis of a university-sponsored company and learning system known as Medical Training Solutions.  He has authored dozens of interviews and editorials about patient safety.  He is a frequent speaker at professional meetings where he lectures on issues related to laboratory test utilization, test interpretation, medical errors, quality improvement, medical informatics, and competency assessment. For his contributions to patient safety, Dr. Astion was awarded the 2006 AACC Annual Award for Outstanding Clinical Laboratory Contributions to Patient Safety. In 2007, he was awarded the Annual Canadian Traveling Lectureship from the Canadian Society of Clinical Chemists. 

Presenter:  Lucia Berte

Lucia Berte is a consultant for quality system implementation in the health system arena, preparing and presenting training programs on quality system implementation in laboratory medicine, blood banking, and clinical systems. She has many years of experience as a laboratory quality assurance director and transfusion service manager, and has extensive experience as a workshop presenter, teleconference speaker, and author. Lucia is certified as a medical technologist, a specialist in blood banking, and diplomate in laboratory management by the Board of Registry of the American Society of Clinical Pathology (ASCP). She is also certified as a quality auditor and quality manager by the American Society for Quality (ASQ). For her contributions to patient safety, Ms. Berte was awarded the 2007 AACC Annual Award for Outstanding Clinical Laboratory Contributions to Patient Safety.

 

 

 

Module 2: Beyond MRSA: The strategic value of the laboratory in the control of healthcare associated infections

Session Summary:

Healthcare associated infections cause more than 90,000 deaths each year in the United States at substantial healthcare costs.  Many of these infections are preventable.  Laboratories are being asked to assume a more active role in infection management through performance of active surveillance, outbreak investigation, performance improvement initiatives and other tasks in an era of shrinking resources.  This audio conference will address strategies beyond MRSA detection and highlight the role of the laboratorian in multidisciplinary activities that enhance patient safety and contribute to prevention of healthcare associated infections.  

 

Learning objectives:

At the conclusion of this presentation the participants will have learned:

  • The impact and scope of healthcare associated infections.
  •  The importance of the microbiology laboratory in infection control and antibiotic management strategies.
  • How to improve leaderships' view of the importance of the laboratory in patient safety initiatives as they relate to healthcare associated infections.

Presenter: Karen Carroll, M.D.

Dr. Karen Carroll received her MD degree from the University of Maryland School of Medicine where she also did an internship in Internal Medicine. She completed her residency in Internal Medicine at the University of Rochester Associated Hospitals Program and a fellowship in Infectious Diseases at the University of Massachusetts Medical Center.  Dr. Carroll then relocated to Salt Lake City where she completed a fellowship in Medical Microbiology and in 1990 became the Assistant Director and then later the Director of the Infectious Diseases Laboratories.  In 2002, she became the Director of the Division of Medical Microbiology at the Johns Hopkins University School of Medicine. Her research interests include evaluation of diagnostic platforms for health care associated infections. She is an active member of several Infectious Diseases, Microbiology and Pathology Professional Societies and Committees.

 

Module 3: Benchmarking for Patient Safety
Wednesday, October 29, 2008                3:00 PM – 4:30 PM, ET

Session Summary:

This conference is designed to update the listener on the current status of efforts to benchmark patient safety and quality practices in clinical laboratories and resources available to laboratory professionals.  In addition Sigma Metrics as a benchmarking tool will be discussed.  The focus will be on practical application of benchmarking in the laboratory

Learning Objectives:

At the conclusion of this audio conference the listener will have a greater understanding of:

  • The current state of evidence supporting what is considered to be quality laboratory practices
  • Resources available for benchmarking
  • The use of Sigma metrics for benchmarking in the laboratory 
  • Practical application and use of available benchmarking tools

 

Presenter:  Teresa Darcy MD, MMM

Teresa Darcy MD, MMM, is Medical Director of Clinical Laboratories for the University of Wisconsin Hospital and Clinics and serves as Medical Director for the University of Wisconsin Clinical Laboratory Science (CLS) Program.

Dr. Darcy received her BA degree from Carleton College in Northfield, Minnesota and MD from the University Of Iowa College Of Medicine. After completing a residency program in Anatomic and Clinical Pathology at the Naval Hospital, San Diego, California she completed Fellowship training in Cardiovascular Pathology at the Armed Forces Institute of Pathology in Washington DC. Dr. Darcy is Board certified in Anatomic and Clinical Pathology. She received the Master of Medical Management degree from the University of Southern California, Marshall School of Business.

Dr. Darcy served as the Chief of the Pathology and Laboratory Medicine Service for the VA Hospital in Des Moines, Iowa, the Director of Clinical Pathology for St. Joseph Hospital at Creighton University, Omaha Nebraska and Director of Core Laboratories for the Methodist Health System, Omaha Nebraska before joining the University of Wisconsin faculty. Her experience in clinical laboratories over the past 20 years includes labs in 10 bed Critical Access Hospital Labs, Physician Office Labs, Point of Care Programs, Outreach lab services and multifacility University based teaching hospitals.  

Dr. Darcy serves as a member of the CLMA Patient Safety and Quality Committee.

 

Module 4: National/International Strides to Patient Safety and Quality

Session Summary:

Improving patient safety and quality within the diagnostic industry is a major focus of every laboratory leader.  This session will provide an overview of the key initiatives underway within the United States and Internationally.  Included in the discussion will be such topics as JCAHO’s National Patient Safety Goals,  Pay for Performance and ISO 15189.  Strategies organizations are deploying utilizing a system approach along with lessons learned along the way will be highlighted in the discussion.

At the conclusion of this presentation the participants will have learned:

  • To provide an update on Patient Safety and Quality key initiatives Internationally and within the United States
  • To learn strategies that will enhance  systems thinking to benefit a laboratory operation on major issues related to Patient Safety and Quality

Presenter:  D. Joe Boone, PhD

D. Joe Boone, PhD, is the Acting Director, Division of Laboratory Systems of the Centers for Disease Control and Prevention. He currently coordinates efforts to enhance America's health by improving the quality of health laboratory services and patient safety, and genetics projects. He works with ISO, OECD, and EuroGentest on international standards, and serves on several Clinical and Laboratory Standards Institute (CLSI) committees. He was the first recipient of the Ortho Clinical Diagnostics sponsored American Association for Clinical Chemistry Award for Contributions to Patient Safety. His interests include laboratory medicine, public health, and health services research.

Presenter:  Anne Daley, MS, MT(ASCP)DLM, CLC(AMT), CMQOE(ASQ)
Present Responsibilities

Ms. Daley is a Senior Managing Consultant for ASCENDIUM.  Her area of specialization includes quality management systems, staffing structures, leadership development, laboratory integration and design, and process optimization. 

Experience

As Practice Lead for Organizational and Operational Improvement Professional Services within ASCENDIUM, formerly Healthcare Solutions from Roche, Ms. Daley has either lead or supported projects with the following key outcomes:

  • Assisted multi-hospital system to in-source their laboratory after 9 years of out-sourcing.  Performed operational assessments and developed new organizational structures.
  • Increased Phlebotomy productivity by 38%, reduced staffing by 17.5% (over $250,000 savings) while improving morning phlebotomy turn-around-time by over 10%.  Consolidated Phlebotomy and Accessioning departments for a large joint-venture, hospital based, core laboratory.
  • Reduced testing turn-around-time for a large plasma fractionation testing laboratory from 34 days to 14 days, a 59% reduction within 3 months.
  • Managed a core laboratory redesign and implementation that reduced overall lab space by 23%.
  • Provided a Strategic Directions Program that included Change Leadership training workshops and individual Business Coaching sessions, significant gains in productivity and job satisfaction.

Prior to becoming a consultant, Ms. Daley led integration and process optimization efforts for a large multi-hospital and commercial laboratory network, holding three key roles over 8 years: 

  • As Executive Director, Technical Operations for Laboratory Sciences of Arizona and Sonora Quest Laboratories, responsibilities included Banner Health’s Arizona laboratory network.
  • As Executive Director Hospital and Commercial Technical Operations, responsible for over 650 FTE’s and $60 million in total expense.
  • As Director Hospital Operations and Decision Support Services, provided leadership for Samaritan Health System’s five hospital laboratories and developed system support functions.

Previous positions also included; Administrative Director for Maricopa County’s laboratory system that included a core laboratory and 13 clinics, Director of Ancillary Services (Radiology, Pharmacy, Laboratory) for a rural community hospital which was part of Aurora  Health Care, and Laboratory Supervisor for a 12 physician internal medicine group practice laboratory.
